Absence Request Form

As stated in our school attendance policy, only exceptional circumstances warrant an authorised leave of absence. Generally, a need or desire for a holiday or other absence for the purpose of leisure and recreation would not constitute an exceptional circumstance.

If seeking permission for exceptional circumstances leave of absence, the parent/guardian should complete a Leave of Absence Request form. The request should be submitted as soon as it is anticipated; and wherever possible, at least four weeks before the absence. Leave of absence cannot be approved retrospectively.

The school will consider each application individually, considering the specific facts and circumstances. Parents may be required to provide the school with additional evidence in order to support the request. The expectation of the school is that term time holidays should not be planned or booked as a matter of course, as they are likely to be unauthorised.

Where a leave of absence is granted, the school will determine the number of days the pupil can be away from school. A leave of absence is granted entirely at the school’s discretion.

In April 2017 the Supreme Court clarified the definition of regular attendance to be attendance “in accordance with the rules prescribed by the school”, therefore if an absence is not authorised by the school, the pupil’s attendance is deemed to be irregular.

Administration of Medicines - Parental Agreement Form

Please complete this if you need school to administer medicine to your child.  We will not be allowed to administer anything until we have this form in the school office. We can only administer medicines from  a prescription issued by medical doctors who are registered with the GMC (general medical council).

Long Term Collections & Permission To Walk Home Alone 

The school never releases a pupil into the care of another adult who is not a family member, named emergency contact or named long term collection contact without the consent of their parents. We will also never release a pupil to walk home alone, unless prior consent is given by the parent. 

If you would like to give consent for your child to go home with someone else or to allow them to walk home independently, please ensure that you follow the outlined procedures below:

ONE OFF COLLECTIONS
One off collections like a play date, can be emailed or called through to the office on 0117 3772610 by 2.30pm at the latest.  Please ensure whoever is collecting your child is able to show some form of ID if they are not a known adult to the class teacher. 

LONG TERM COLLECTIONS 
If you wish for your child to be collected by another adult on a regular occurrence, you must complete this form. Please ensure whoever is collecting your child is able to show some form of ID if they are not a known adult to the class teacher. 

PERMISSION TO WALK HOME - Years 5 & 6
There is no set age when children are ready to walk to school or home on their own. It very much depends upon their maturity and confidence. We believe that you as parents need to decide whether your child is ready for this responsibility. However, we would still highly recommend that pupils in year 4 at least are still brought to and collected from school. In deciding whether your child is ready to walk to school, you should assess any risks associated with the route and your child’s confidence. If you give permission for your child to walk home from school, you must complete this form.
